Comprehensive Implant Revision with Downsizing, Mastopexy, and Internal Bra
A patient in her 60s presented after years of discomfort stemming from large, 600 cc silicone implants placed by a different surgeon. She suffered from painful capsular contracture, a confirmed implant rupture, and significant breast ptosis (sagging). The sheer weight of the oversized implants caused a downward pull, resulting in chronic discomfort and an unnatural, heavy appearance.
To resolve these issues, she underwent a total breast revision, including implant removal, subtotal capsulectomy, and replacement with significantly smaller 360 cc silicone implants. To reshape and support the new, smaller profile, a formal breast lift (mastopexy) was performed, reinforced with an internal bra support matrix.
Shown here more than one year after surgery, the patient is pain-free and thrilled with her result. She has achieved a softer, lighter, and more youthful breast shape that perfectly suits her frame. Her scars have matured excellently, becoming virtually unnoticeable. This case highlights how combining surgical techniques can successfully treat rupture, contracture, and sag while restoring a patient's physical comfort and confidence.
