Revision Breast Augmentation for Implant Malposition and Animation Deformity
This 29-year-old patient came to Dr. Tandon four years after a subpectoral breast augmentation with 250cc implants performed elsewhere. Her implants had migrated laterally toward the armpit and settled too low on the chest, leaving her with animation deformity, poor cleavage, and a result she was not happy with.
The problem: Subpectoral (under-the-muscle) implants rely on the pectoralis major to hold them in position. When that muscle isn't properly repaired or the pocket is made too large, implants can shift laterally, drop below the natural fold, or flatten and distort every time the chest muscle fires. This patient had all three.
The correction:
- Removal of both implants intact
- Subtotal capsulectomy to clear scar tissue from the original pockets
- Repair of the pectoralis major muscles back to the chest wall
- Conversion from a subpectoral to a prepectoral (subfascial) plane — moving the implants above the muscle so they no longer move with it
- Placement of Motiva SmoothSilk RSF 375cc silicone implants
- DuraSorb bioabsorbable mesh for lower pole support, to hold the new implant position while her own tissue strengthens around it
At two months out, her implants sit centered and symmetric, her cleavage is restored and improved, and animation deformity is gone — because the implants are no longer under a muscle that can displace them.
